Pendular See-saw Nystagmus as a Delayed Complication of Traumatic Brain Injury

Abstract
A 54-years-old man presented with oscillopsia for 9 months. He had sustained head trauma due to a motor vehicle accident 15 years before. Examination showed bilateral optic atrophy, and pendular see-saw nystagmus that markedly decreased during convergence. Brain MRI disclosed encephalomalacia of both frontal lobes, hydrocephalus, and atrophic optic chiasm. Pendular seesaw nystagmus may be a delayed complication of traumatic brain injury. Suppression of pendular see-saw nystagmus during convergence may be explained by an adaptive mechanism for the increasing diplopia due to seesaw nystagmus while near viewing.
